Are you a heart failure patient or caregiver? Do you want to join our patient and public advisory group to help us create a digital version of REACH-HF?

We are creating a digital version of the REACH-HF programme to improve patient choice of, and accessibility to, cardiac rehabilitation.

We hope to engage individuals living with heart failure and caregivers in the design, delivery and dissemination of this project to ensure that the work undertaken is sensitive to the needs of HF patients.
